Functional & Workplace Evaluations
I provide objective, clinically-informed functional assessments to evaluate an individual’s ability to function in daily life, work environments, and under stress. These evaluations are often used to support disability claims, workplace accommodations, or legal cases, providing clear, professional documentation to inform decision-making.
Who This Is For
This service is for individuals, employers, attorneys, or HR professionals seeking structured clinical insight into functioning. Evaluations can help determine workplace readiness, document disability-related needs, or provide objective information for legal or employment purposes. All assessments are conducted by a licensed clinical social worker with experience in mental health, trauma, and functional assessment across diverse populations.
Types of Cases Supported
I provide mental health evaluations for:
Workplace Evaluations (ADA accommodations)
Disability Evaluations
Fit-for-Duty Evaluations – safety-focused assessments for employees in high-risk roles to determine if they can perform duties without risk to themselves or others
These evaluations may include reviewing work history, functional capabilities, and mental health considerations, resulting in an objective report for employers, supervisors, or other relevant parties.
Why Functional Assessments Matter
Functional assessments provide more than just documentation, they offer a structured, objective perspective on daily and work functioning. Employers, attorneys, and individuals benefit from understanding an individual’s strengths, limitations, and needs, which can guide workplace decisions, accommodations, or legal proceedings.
What to Expect
Each assessment is tailored to the individual and may include a clinical interview, history review, evaluation of daily and work functioning, standardized screening tools, and behavioral observations. A professional report is provided with objective findings, clinical impressions, and actionable recommendations suitable for legal, medical, or employment use. The process is straightforward: schedule the evaluation, complete intake forms, attend the virtual assessment, and receive your completed report (within 10 business days.)
Telehealth Services
All evaluations are provided exclusively via secure, HIPAA-compliant telehealth. Evaluations are available to residents of California and Indiana and can be accessed via computer, tablet, or phone. Please note that in-person sessions are not offered.
